CARES - Coordinating Access to Resources & Essential Services Program

The CARES (Coordinating Access to Resources & Essential Services) Program is designed to connect parents and guardians like you with tools, programs and services through free, short term resource navigation services to help families access the tools and support they need. We serve parents and guardians of children ages 0-18.

(Available in Clay, Dickinson, Emmet, Kossuth, Osceola, O'Brien, Palo Alto and Winnebago counties.)

What is Resource Navigation?

Resource Navigation is a free and supportive service that helps individuals and families connect to a variety of programs, resources, and services. Resource Navigators are trained professionals who work directly with you to understand your needs and guide you toward the right help:

  • Listen to your specific needs

  • Identify available resources in your area (government programs, nonprofits, support groups, etc.)

  • Explain how to qualify and apply for those resources

  • Help with paperwork, applications, or referrals

  • Follow up to make sure you’re getting the help you need

How do Resource Navigators Help?

  • Connection and Referral to Resources

    • Resource Navigators can help individuals access financial assistance, emotional support, and informational resources available in the community.

  • Financial Concerns

    • Resource Navigators can provide budgeting tips and worksheets to help you manage your finances more effectively.

  • Employment/Volunteer/Higher Education Opportunities

    • Resource Navigators assist individuals in completing applications for employment, unemployment, and Social Security benefits. They also help connect people with volunteer opportunities and educational programs within the community.

  • Food Insecurity

    • Resource Navigators can guide you to food pantries, nutrition programs, and other essential services.

  • Behavioral and Physical Health

    • Resource Navigators can help individuals find a primary care physician and access counseling, therapy, and support groups.

  • Healthcare Coverage

    • Resource Navigators can help individuals apply for Medicaid, Hawk-i, and explore other healthcare options.

What is Required for Program Eligibility?

Our Resource Navigation Program is designed to support families within our community by connecting them to essential services and opportunities.

Eligibility Requirements

To qualify for Resource Navigation services, applicants must:

  • Be part of a family unit, including single parents or guardians with children up to age 18.

  • Reside within the designated service area of our community outreach office, which includes the following counties:
    Clay, Dickinson, Osceola, O’Brien, Palo Alto, Emmet, Kossuth, or Winnebago.

Priority is given to families who meet any of the following criteria:

  • Family income at or below 200% of the Federal Poverty Level (FPL)

  • Parents with a high school diploma or less

  • Children with an IFSP (Individualized Family Service Plan) or IEP (Individualized Education Program)

We are committed to equitable access for all eligible families, ensuring those most in need receive the support and resources necessary to thrive.
